Trinity Heart and Vascular Group PLLC Website Privacy Policy

Privacy Terms and Conditions

Trinity Heart and Vascular Group (also known as “THVG”) continually strives to protect your privacy and security when you visit our website. When you gain access to our website, you are agreeing to this policy, so please read it carefully before using or interacting with any part of our site.

When using our website, you consent to allow Trinity Heart and Vascular Group to collect certain types of basic information, and you give us permission to use and possibly share that information with necessary third parties. We do not sell your basic information (such as your email address or phone number), but we may use it to communicate with you, and also use it to enhance our marketing and advertising campaigns.

Types of Information That We Collect Through the Website

You are not required to interact with our website, meaning, you do not have to fill out any contact forms, or call us or email us. You are allowed to use our website to gain information about our practice, and for educational resources, and are not obligated to give us any personal information.

If you do decide to communicate with us directly through the website (via contact forms, the online appointment scheduler, or by clicking on links to email or call us), you may be asked to provide us with your first and last name, email address, phone number, mailing address, and other general information.

Any information that could potentially be public knowledge or found online (such as your name, phone number, and address in an online phone book) is defined as “personal information.”

If you decide to send us any personal information about yourself, you give THVG permission to receive, transfer, and store that information, and to also use it in relation to the operations of our cardiology practice, including communicating with you.

If you send us personal information about another individual, you represent that you have received that person’s express permission to do so, or you are designated as their POA if they are unable to communicate or make their own health decisions. If representing another individual, you give us permission to use their personal information in accordance with our privacy policy.

Details about the Security of Your Data Through Our Website

THVG desires to protect your privacy when you use our website, and we make reasonable efforts to protect your data from hackers. We utilize a ‘TSL/SSL’ certificate (also known as a “Transport Layer Security” certificate or “Secured Sockets Layered” certificate), which provides a secure connection to our website.

The SSL certificate encrypts communication between your web browser and our web server. This means that when you supply us with personal information, such as your name and contact information, that information is locked so that unauthorized users can’t see it.

[To verify that we have this certificate, a padlock icon should be seen next to our domain name in your web browser.]

Even though we do our very best to protect your personal information, THVG cannot guarantee that the information you send through our website will be 100% secure, as there are rare circumstances where hackers may be able to unlock or breach our secure connection. If you choose to send us personal information, you are assuming the risk that comes with that action.

Since we cannot 100% guarantee that your personal information is secure at all times (such as your name and email address), you agree to not send private information through our website contact forms. We define “private information” as any information that someone could use to either steal your identity and/or compromise your health privacy.

When using our website, you agree to not submit your credit card numbers, social security number, personal health records, or any other type of information that shouldn’t be seen by the public. If you do so, you assume full responsibility for the risks involved. If you wish to email us private information, you can contact our office and request to set up an account to send and receive HIPPA-compliant emails.

Types of Information That We Collect Through Our Online Scheduling Software

Trinity Heart and Vascular Group uses ‘Jane App’ scheduling software, where patients can schedule their own appointments online. Through the Jane website, we obtain information that includes (but is not limited to) your first and last name, contact information, payment information, and reason for appointment.

Details about the Security of Your Data Through Third-Party Applications

The Jane App: When you click on the online scheduling link through our website, a new window will open that takes you to Jane’s website (https://jane.app).

Jane Company is SOC 2 Type 1 Certified by the American Institute of Certified CPAs. This means that they are certified to handle private financial data online. To learn more about Jane’s policies regarding security and privacy, please go to https://jane.app/security-and-trust.

When you schedule an appointment through Jane, THVG’s Terms of Use and Privacy Policy do not apply to your usage of the Jane online scheduler. Once you arrive at Jane’s website, you are agreeing to their terms of use and privacy policies. You agree that TH&VG is not held liable for any security breaches that may happen through Jane’s website.

EnGuard Email: We use EnGuard for our HIPPA-compliant email services.

When you officially become a patient at THVG, and you properly set up an account, you and your provider can send and receive HIPPA-compliant emails. This means that you can send and receive PHI in a secure setting.

EnGuard’s email services are connected to our domain name and web host, but we do not store your EnGuard emails in our website files, as Enguard has their own separate server.

When you set up an account through EnGuard, you are agreeing to their terms and conditions. As such, you are agreeing that THVG will not be held liable in the unlikely event of a security breach in EnGuard’s database.

You assume full responsibility for the information that you send, and you understand that there is always a risk, even if it’s highly unlikely, that your private health information could be breached.
